The high revs is the choke warming up the truck. When you tap the gas it is kicking off the choke and going to too low of an RPM for the cold engine so thats why it sounds like it is running rough. Just my guess.

And reason for the cloud of white smoke at the beginning?

Could be just condensation if white smoke. If grayish, that is oil. You can try replacing just the valve seals and see if it disappears so you don't have to pull out the head. If it does not disappear, it's valve stems and the head needs to be rebuilt.

and also depending on the size of the crack, you typically will not see white smoke on start up. if it is hairpin crack it will start up normally then when the head gets warm the crack will open cause of the metal expanding therefore letting coolant in...but if it is a good size crack, then it will smoke right off when in starts and will keep smoking

if it is just smoking on start up but goes away when at running temperature, white smoke just means condensation in the exhaust
