wheel adapters

Got a question, I know in here I will get my answer.
I want to use a set of 17" wheels that I had on my 93 Chevy Astro van and put'em on my 95 toy pick-up, What adapters do I need?

something that will convert from gm (5 x 5.00 ?) to 5 x 4.5 or 114.3.

gm is 5x4.75(on an astro/s10)

so where is a good place I should try to get.em
