Rear Brakes and Wheels

Does anyone make a kit to convert rear drum brakes to disc brakes for a 1984 Mighty Max?
How can I convert the rearwheels from 6 lug to 5 lug?

Don't know of a kit but the easiest way to do this I can think of would be to swap a rear axle assembly from a late 80s to early 90s Toyota Pickup. There's a write up in the tech section of this site how to do a disc brake swap on a Toyota axle.
