a little PB blaster and a lot of patience. One time I had to put the crank pulley back on the timing bolt, loosely (3 bolts, about 3-4 turns in,) and used the crank pulley like a slide hammer.

the "bolt" on the end of the crankshaft, does it come out or is it just there to use a socket to turn the crankshaft?

That big bolt has to come off. It's right-hand thread. Use an air wrench, or put the truck in 4th gear, put on the e-brake, and use a breaker bar with a socket. You might need a pipe on the end of the breaker bar for more leverage. Once the bolt is off, the gear should slide off easy. If not, a small claw bar should do it.
